We will work directly with the automobile insurer and/or legal team to ensure that the client is provided with the necessary Medical/Legal reports to address their specific home accessibility requirements.Different types of collaborative assessments and reports are prepared by our regulated health care providers and build experts. You may require one or all of the following reports:
Barabco AccessAbility will provide a comprehensive report which identifies the existing barriers within the pre-accident home, specific to the client’s impairment. Our report will detail the necessary barrier-free modifications needed to provide a safe and fully accessible home for the client’s current and future needs. Our reports include information and pictures about the existing home, detailed description of the required modifications, architectural floor plans, computer assisted 3-D rendered images of the proposed modifications along with associated cost charts for the proposed work.
In some situations it may be necessary to perform a home site assessment immediately in order to prepare a comprehensive report detailing the necessary home modifications required for your client's safety and accessibility on a short term basis. For example if a patient is being released from the hospital to move back to his/her home, short-term or temporary home modifications would have been assessed and implemented in order to ensure a safe and accessible temporary living environment. A more detailed complete Home Accessibility Report for permanent modifications will have to be done at a later date.
These reports are prepared in circumstances where our expert team has concluded based on our Home Accessibility Report that your client's current home is either unfit for the required home modifications or the cost of implementing the home modifications would exceed the cost to purchase and remodel a new barrier-free property. In such scenarios, Barabco AccessAbility will prepare a collaborative report with our Real Estate partner from Slavens and Associates Realtors to provide a detailed list of both new and re-sale properties that would be appropriate for your client's special needs and would enable the required home modifications. The Alternative Housing report is a comprehensive report which lists the potential properties and the modifications that would be required as well as the anticipated cost for all options.
A detailed and objective evaluation of another provider’s assessment of a client’s required home modifications. This assessment report can be provided by way of a paper review or an In-Person assessment. The focus of the review is to provide commentary on the need for the proposed recommendations and to provide detailed costing in order to determine whether or not the proposed cost for the home modification is reasonable.
In instances where a home modification assessment has already been performed but questions arise with respect to the scope of the proposed modifications and/or the cost of the proposed modifications, a second opinion is required. We will prepare a comprehensive report which identifies the existing barriers within the pre-accident home, specific to the client’s impairment. Our report will detail the necessary barrier-free modifications needed to provide a safe and fully accessible home for the client’s current and future needs with all associated costs involved.
